After completing the registration process you will download and print 4-6 pages of information including the following:
- Your completed Voter Registration/Ballot Request form, ready for your signature and date.
- An Addendum page to accompany the form, ready for signature and date. Do not submit this page if it is blank.
- A personalized letter of instructions, which includes the mailing address for your form and addendum.
